Actually this is kind of a reasonable comment. We know that Hamas launched rockets and we can make (ideological) conjectures as to why they might have taken this action, but there was no specific incident that triggered the rocket launchings. I believe that's what she's getting at.
Institutionally, it is called 'interpretation bias'... but for those of us who simply use common sense, it is usually called: head-up-your-butt-itis.
Seriously, 'interpretation bias' is where, no matter how strong the facts or data, if it is not confirmation of expected results or worldview, it doesn't exist, or is invalid. "Science" has this just as bad if not worse than the LSM, and everyone has this to a certain extent.
In plain english: if what you see doesn't tell you that you are right, you immediately assume that the 'evidence' is wrong, no matter how stupid it makes you look. You can't see what you don't want to, and only see what you expect to.
ie: "It's those dang jews who caused this war!" or "Man-made global warming is the greatest threat to mankind! - Sea levels are so high that we're already underwater and don't know it yet!"

"There are none so blind as those who refuse to see." This is the first thing which came to mind when I heard Ms. Maddow's ignorance of what is transpiring between Israel and Palestine and when I looked up the origin of the quote it turns out it is a paraphrase of Jeremiah 5:21
"Hear now this, O foolish people, and without understanding; which have eyes, and see not; which have ears and hear not."
When you think about how long ago Jeremiah wrote this verse it is astounding with as "smart" as mankind thinks they have become they are as stupid as they ever were.
